E Parent父母/Guardian’s Agreement and Declaration监护人的协议和声明
Being the Parent/Guardian of the student mentioned above, I do hereby agree to undertake with CWA as follows:
作为学生的父母/监护人, 我特此向长沙玮希国际学校作出如下保证:
1.That the above information provided and any materials submitted along with the application is complete, true and correct to the best of my knowledge. If it is later found that essential details have been withheld or are untrue at the time of application, the school reserves the right to withdraw acceptance of my child and I shall not ask for any refund of the fees paid, if any. I shall also undertake to pay my company for the loss if these amounts are sponsored by my company. I also understand and agree to the school’s Withdrawal and Refund Policies.
本申请书所提供的信息及所有资料都是至今为止最完整、最真实和最准确的。如发现在递交申 请表格时有故意隐瞒重要细节的或填写不确实的，学校有权勒令我的孩子退学，在此情况下，我就不能要求学校退回任何已缴的费用。如果所缴费用是我的公司赞助的，我将自行承担公司的损失。同时，我已了解并同意学校所公布的退学及退款条例。
2. That I have read and fully understand the terms and conditions listed in the CWA Fee Schedule and CWA Admission Policy and Procedures for the current academic year attached herewith, and that school fees shall be paid before my child can attend the school. Whether the fees are paid by the family or by the company, I shall take full responsibility to ensure tuition is paid on time in preventing my child from being unable to attend school classes. (I understand that the CWA Fee Schedule will be reviewed annually and parents will be notified officially.)
我已经阅读并完全了解长沙玮希国际学校关于本学年的《收费明细条款》和《入学政策与程》，同时我也保证将会在孩子入学之前缴清所有款项。为确保我的孩子能够正常上课，无论是我个人或是公司付费，我将负责保证所有学费款项都按时缴纳。（我也明白学校将每年会复核《收费明细条款》，并将会以书面形式通知家长。）
3.That it is my responsibility to notify the school in writing immediately of any changes of information, e.g. company/job, address, contact number(s) and my child’s medical health, etc.
我承诺我或我的孩子若有任何信息变更，（如：公司/工作，地址，联系电话及孩子的医疗健康 信息等等），我将在最快的时间内，以书面形式通知学校。
4.That my child will attend the school regularly and shall strictly conform to the Rules and Regulations adopted by the school, including any mandatory drug testing policy.
我的孩子将按时上课，严格遵守学校各项规章制度，包括毒品测试条例。
5.That I will be responsible to ensure my child has the approval to enter and remain in China with a valid visa and passport, otherwise I would have to remove my child from the school.
我有责任确保自己的孩子拥有在中国居留的合法证件和护照，否则我的孩子将自动离校。
6.That the school may at its absolute discretion require the removal/suspension of my child with minimum notice if he/she has been guilty of serious misconduct (such as bullying, physical violence, etc.) In this case, I shall not ask for any refund (or reduction) of any portion of any school fees already paid (or payable) to the school.
如果我的孩子有严重的违规和不正当的行为，如：恐吓，打架等，我认可学校有权裁决让他/她在最短的时间内退学或停学。在这种情况下，无论是已缴学费（或任何需缴费用），我都不能索求退款（或是要求任何减免）。
7.That photographs or videos of my child may appear in school materials, including but not limited to CWA brochures, websites, advertisements and newsletters.
我同意孩子的照片或视频可能会出现在学校或与学校有关连的材料上，如：宣传册，网页，广告及简报等。
8.That I agree although the school will take every precaution to ensure the safety of all students, accidents do happen. Thus the school will in no case be held responsible for any injury that may occur to my child whilst attending the school or whilst traveling to and from the school.
我明白长沙玮希国际学校会尽量采取一切措施来保护学生的安全，但事故仍然有可能会发生。因此，如果我的孩子在学校里、出游时、上学或放学途中发生的意外事故中受伤，我不会指责学校、或追究学校的责任。
